How we created professional medical portraits in a challenging operating theatre environment

Working in an operating theatre presents unique lighting challenges for photographers. The space is designed for surgical precision, not photography, which means we need to bring our own lighting solutions while respecting the sterile environment and working around complex medical equipment.

The Challenge: Lighting in Theatre

When Dr MacKay requested photos in the operating theatre at Epping Private Hospital, we knew we'd be working in one of the most technically demanding environments for photography:

  • Existing fluorescent lighting creating harsh shadows and unflattering colour temperature

  • Reflective surfaces everywhere - from surgical lights to stainless steel equipment

  • Limited space to position lighting equipment around surgical instruments

  • Sterile environment requirements limiting where we could place gear

Our Lighting Solution

Key Light: Umbrella Setup We used a single umbrella setup positioned to camera left, providing:

  • Soft, diffused light that eliminates harsh shadows

  • Even illumination across the subject's face

  • Professional quality lighting that complements the clinical environment

Why Umbrella vs Softbox? In the confined theatre space, umbrellas offered several advantages:

  • Faster setup - crucial when working around busy surgical schedules

  • Compact storage - easier to navigate around equipment

  • Softer light spread - perfect for environmental portraits

Technical Considerations

Power and Positioning

  • Used battery-powered strobes to avoid cable hazards in the sterile environment

  • Positioned lighting to avoid interference with surgical equipment

  • Balanced strobe output with ambient theatre lighting for natural-looking results

Camera Settings

  • Shot with professional mirrorless system for silent operation

  • Balanced exposure to retain theatre environment details while properly exposing subject

A flash with a diffuser umbrella to light the surgeons from beneath

The Results

The lighting setup allowed us to capture:

  • Professional headshots with soft, flattering light on Dr McKay

  • Environmental context showing the sophisticated surgical suite

  • Authentic atmosphere that conveys the importance and precision of surgical work

  • Consistent quality across multiple shots and poses
